An account that does not exist counts as having nonce `0`. The table includes `tx.sender` whenever `APPROVE` increments its account nonce in the validation prefix, and also includes any account that executed `CREATE` or `CREATE2` and any contract the transaction created. If a transaction's replay protection is kept in contract storage instead of the account nonce, `APPROVE` writes that storage, and the write appears in `slots_changed` rather than in `nonces_changed`.
